Throughout the school year, parents can access all necessary school forms and pertinent school information below.  Families can also update contact information and preferences by logging on to Parents Web using the link to the right of this page. Contact the Front Desk with any questions..

Required school forms

All required Back-to-School Forms are to be completed electronically through the secure ParentsWeb system.  Please log onto ParentsWeb to find required forms under the Web Forms menu.  All electronic forms need to be completed by the first day of classes.  These forms include the following:

Family Demographics Form, which includes:
  • PCDS Student Medical Form
  • PCDS Parent Contact Information
  • PCDS Student Contact Information
  • Transportation/Pick-Up Authorization Form

Other Back-to-School Required Form, which includes:

  • PCDS Student Permissions/Waiver Form
  • Family Handbook Agreement
  • Volunteer Driver Form
