School Forms & Resources
Throughout the school year, parents can access all necessary school forms and pertinent school information below. Families can also update contact information and preferences by logging on to MyPCDS online community, using the link to the right of this page. Contact the Front Desk with any questions.
Required school forms
All required Back-to-School Forms are to be completed electronically through the secure MyPCDS
online community. Please log onto MyPCDS to find required forms in the Back-to-School Forms item on the Resource Board. All electronic forms need to be completed by the first day of classes. These forms include the following:
- Student Medical Form
- Permissions/Authorizations/Waivers Form, which includes:
- Family Handbook Agreement
- Volunteer Driver Form
- Technology Use Policy & Agreement
